  • Washington Integrated Care Assessment

    Diana is working with Washington State’s Health Care Authority, its nine Accountable Communities of Health, and five Managed Care Organizations to implement a statewide integration assessment that will help providers better serve their patients by addressing both physical and behavioral health needs.

    Kindergarten readiness metric for the health care system

    Diana partnered with the Children’s Institute, the Oregon Health Authority, and the Oregon Pediatric Improvement Partnership to convene a workgroup on how the health sector can help prepare children for kindergarten. The 17-person group included primary care providers, researchers, child advocates, and leaders at early learning programs. After nine months of discussion and deliberation, the workgroup agreed on new measures of success for Oregon’s Coordinated Care Organizations.

    Advance Directive Advisory Committee

    Over two years, Diana worked with a multi-stakeholder group including lawyers, health care providers, hospital representatives, and ethicists to draft a new advance directive form for Oregon. The legislature adopted the new form in its 2021 legislative session.

  • Oregon’s Value-Based Compact Workgroup

    Diana works with the Oregon Health Leadership Council and the Oregon Health Authority to convene and staff a 15-member workgroup of payers and providers tasked with implementing a voluntary compact that will facilitate value-based payments throughout the state.

     

    Comprehensive Primary Care Payer Group

    For five years, Diana helped 15 health systems and Coordinated Care Organizations (CCOs) meet the requirements of a federal initiative to transform the payment and delivery systems of primary care. The payers (many of whom are competitors) identified opportunities for alignment and shared learning, as well as strategies for transforming the delivery system to better serve patients. Our partners on this project were the Oregon Health Leadership Council and Comagine Health. 

    Primary Care Payment Reform Collaborative

    Since 2017, in partnership with the Oregon Health Authority, Diana has staffed and facilitated the Primary Care Payment Reform Collaborative, a body created by legislators to increase investment in primary care and to improve reimbursement methods.

  • Washington State’s Accountable Communities of Health

    Since 2019, Diana has teamed with two other consultants to convene and staff a collaborative of the leaders of Washington’s nine Accountable Communities of Health (ACHs). The ACHs are an integral part of the state’s Medicaid transformation efforts, and the consultant team has helped the group identify common interests and projects to improve the health of Washingtonians.

    Oregon Primary Care Association

    For more than 10 years, Diana has organized and facilitated the Oregon Primary Care Association’s annual strategic planning retreat for community health clinics around the state. Diana works with the staff and board to design a meaningful process, identify organizational priorities, and conduct a productive and inclusive retreat.

    Project Access NOW

    Project Access NOW seeks to improve the health and well-being of communities by ensuring access to care, services and resources for those most in need. Diana has provided guidance to the organization since its inception and, most recently, helped draft a new multi-year strategic plan.
